I am a Jamaican who got accepted to a college in Canada. My aunt living in Canada will be covering my expenses but she needs to provide an affidavit form to the Canadian High Commission here in Jamaica. How can either of us get that form?

She should be sure to follow its directions if she needs to attach bank documents, for example, and if she needs to have the form certified or notarized, etc., which she probably does if it is an affidavit.</p>
