Cystic Fibrosis is an inherited disease which affects more than 9 thousand people in the UK. It is life threatening, causing severe congestion of the lungs and the digestive system. Over 2 million people carry the faulty gene, accounting for 1 in 25 of the population. The highest rates are among Caucasians, but other ethnic groups are also affected. Every week, 5 babies are born with the disease, and 2 young children die of it; only half of those who survive to adulthood live beyond 40 years of age. Diagnosis is of vital importance in treatment and prevention. Carriers of the faulty gene can be identified with a simple mouthwash test, saliva, or blood test. This is necessary if there is a history of Cystic Fibrosis in the family. In cases where a pregnancy begins with in-vitro fertilization, genetic testing can be carried out before the egg is implanted. Two antenatal tests are available for mothers thought to be at risk. The first is an amniocentesis, which can be carried out between 15 and 20 weeks, and involves a needle being inserted through the abdomen. A little of the amniotic fluid which surrounds the baby is drawn off and tested for abnormalities. The second test is the Chorionic Villus Sampling, which can be done after 9 weeks of pregnancy. The procedure involves inserting a tube through the vagina and into the uterus, to remove a very small piece of the placenta for testing. A general aesthetic is not needed, the mother-to-be can remain awake, and be given local aesthetic if required.

The Guthrie test is carried out on all babies for this, and other inherited diseases, in the first week after birth. This is done very simply with a pin prick on the heel of the foot, which allows a little blood to be collected and tested to check for raised levels of immuno-reactive trypsinogen.

A sweat test is also available, and is used when the newborn screening tests are abnormal, when symptoms of the disease are present, or if there are serious bowel obstructions. This test is considered to be the most accurate, and is regarded as the "Gold Standard" test for the disease. Sufferers have defective sweat glands which result in elevated salt levels; these can be measured with a painless procedure, even on young babies. The whole process lasts about an hour. Skin should be clean and free of moisturising lotions or creams, and extra clothing or blankets used to increase body temperature. A small section of the forearm or thigh is smeared with a sweat inducing gel containing a chemical called pilocarpine. Two electrodes are attached, and a weak current is generated to stimulate sweating. The sweat is then collected onto absorbent paper, and the salt content is measured.

Chest X-rays can also be used to diagnose the characteristic respiratory complications, as can lung function tests. Digestive problems can be detected by using stool sample analysis.
